In high-pressure gas facilities, safety is not merely about wearing PPE or checking boxes on a Permit to Work (PTW). Process Safety Fundamentals (PSF) require every field operator to view their daily rounds as **active maintenance of safety barriers**.
The Bowtie Concept in Real Operations
In the Bowtie model: - **Threats** (corrosion, high pressure, human error) attempt to trigger the **Top Event** (Loss of Primary Containment / Gas Release). - **Prevention Barriers** (corrosion inhibitors, relief valves, mechanical interlocks, LOTO) stand on the left. - **Mitigation Barriers** (deluge systems, ESD trips, fireproofing, evacuation routes) stand on the right to minimize consequences if containment is lost.
Three Crucial Operator Responsibilities
1. **Positive Isolation & LOTO**: When preparing vessels for entry or maintenance, double block and bleed (DBB) or physical spade blind installation is non-negotiable. Valves leak across seats; mechanical isolation prevents lethal high-pressure gas from entering work zones. 2. **Authorized Gas Testing (AGT)**: Atmospheric testing must be done systematically: Oxygen levels ($19.5\% - 23.5\%$), Lower Explosive Limit ($LEL < 0\%$ before hot work), and toxic gases ($H_2S < 10\text{ ppm}, CO < 25\text{ ppm}$). 3. **Interlock & Alarm Verification**: Bypassing an instrument interlock without a formal Management of Change (MOC) turns a healthy barrier into a Swiss cheese hole.
